Overview
Japan recommends about 14 prenatal checkups per pregnancy, and every municipality subsidizes at least that count from public funds (as of 2025-04; confirm current figures with the Children and Families Agency).4 The national average public spending was 109,730 JPY per pregnant person in the April 2024 survey and 113,647 JPY in the April 2025 survey (as of 2025-04).4 The range still spans from about 80,000 JPY in lower areas to about 136,000 JPY in higher areas, with a prefectural gap near 50,000 JPY between Kanagawa (85,765 JPY) and Okinawa (141,457 JPY) (as of 2025-04).4
About 35 percent of municipalities still left a self-pay remainder under public funding in survey reporting (as of 2024-04).5 That remainder is why the same 14-visit course costs different amounts in different wards.
What the vouchers are and who gets them
The 妊婦健康診査 (ninpu kenkō shinsa, "prenatal health checkup") monitors mother and fetus for conditions such as anemia, pregnancy hypertension syndrome, and gestational diabetes.1 The 妊婦健康診査受診票 (ninpu kenkō shinsa jushinhyō, "prenatal checkup voucher") is the municipal ticket that pays part of each eligible visit at the clinic counter.23
Anyone who submits a 妊娠届 (ninshin todoke, "pregnancy notification") at the municipality of residence receives the 母子健康手帳 (boshi kenkō techō, "Maternal and Child Health Handbook") plus the voucher booklet.23 Foreign residents register the same way as Japanese residents, at the health and welfare counter of the ward or city office.
How the voucher system works
The system is simple at the counter and local in its details. The clinic deducts the voucher amount and the patient pays the rest, while the ward decides which items and facilities qualify.23

How to use a voucher at the clinic

Use order is not fixed. What vouchers do not cover

Every 4 weeks until week 24
Early visits run once every 4 weeks from early pregnancy through week 23.12 Every-visit base items are interview, weight, blood pressure, urine test, and health guidance.28
The first-visit panel covers blood type (ABO, Rh), irregular antibodies, anemia, blood sugar, syphilis serology, hepatitis B antigen, hepatitis C, rubella antibody titer, and HIV antibody.28 Early timed items add cervical cancer screening once in early pregnancy, chlamydia antigen by week 30, transvaginal ultrasound in the weeks 20 to 26 window, and HTLV-1 antibody by week 30.8
Every 2 weeks until week 36
Mid-pregnancy visits run once every 2 weeks from week 24 through week 35.12 Timed items in this band include anemia recheck at weeks 28 to 32, blood sugar recheck at weeks 24 to 28, and Group B streptococcus at weeks 33 to 37.8
Echo count in this window varies by facility. Chiyoda subsidizes 4 abdominal ultrasounds per pregnancy as a separate ticket set, so a clinic that scans every visit bills the extras outside the voucher.26
Weekly to delivery
Late visits run once per week from week 36 to birth.12 Timed items add anemia recheck after week 36 and NST after week 36.8
The NST is a 30 to 40 minute lying-down recording of fetal heart rate and uterine contractions.6 Cervical opening and fetal descent checks prepare for birth as the due date approaches.6
When the checkup finds hypertension, diabetes, or another abnormality, tests and treatment for that condition can fall under health insurance even though the routine portion stays voucher-based.8

Why some visits cost more
The first confirmation visit happens before the handbook and vouchers exist, so it is fully out of pocket at about 10,000 to 20,000 JPY (as of 2026-07-04).7 Register early so the subsidy window starts at the second visit.

Ask the counter three questions: which ticket values apply to which visit numbers, how many ultrasounds are separately ticketed, and which clinics and midwife clinics accept the tickets.23
Moving municipalities and using vouchers elsewhere
Vouchers are issued by the municipality of residence and are generally valid only at its designated or contracted facilities.236 On moving, return unused vouchers to the old office and exchange for the new municipality vouchers with handbook plus new-address proof in hand.7
Chiyoda keeps the same tickets for moves within Tokyo but requires exchange of old tickets for new ones for moves from outside Tokyo.2 For out-of-area births such as home-town delivery, pay in full at the counter and claim reimbursement from the home municipality afterward; caps follow the home ward, with a Chiyoda example cap of 14 prenatal plus 2 ultrasound plus 1 cervical screening.26

Good to know
Register early so vouchers cover from the second visit
Handbook and vouchers work only from receipt forward. Pre-issue checkups and the determination visit never qualify, so early notification moves the subsidy window to the second visit onward.23 A week of delay can leave an extra full-price visit on the bill.
Extra ultrasounds beyond the subsidized count are billed separately
Routine clinics scan more often than the 4 subsidized ultrasounds (as of 2026-07-04).7 Each extra scan bills about 2,000 JPY outside the voucher (as of 2026-07-04).7 Ask at booking which visits draw the subsidized scan tickets and which do not.
Vouchers are only valid at locally registered facilities unless your ward says otherwise
A non-contracted facility cannot accept the ticket at the counter even inside the same city.3 Keep receipts and unused tickets and file for reimbursement where the ward allows it. Vouchers cannot be reissued in principle, and a soiled ticket is exchanged only against the soiled original.2
