Mental-Health Disclosure and the Visa-Renewal Question

Mental health visa Japan is the anxious search behind a simple question: does getting treatment for depression, anxiety, or another condition put your residence status at risk.12 For typical work and family statuses, the published answer is reassuring: renewal reviews taxes, conduct, and continued eligibility, not treatment history.

Overview

Japan's renewal review is a national ISA procedure, applied uniformly across the country. It asks whether you still do the activity your status covers, whether your conduct and livelihood are sound, and whether you have met tax and notification duties.2

Nothing in that review asks for a diagnosis, a prescription history, or a doctor's letter. The rest of this guide shows where that conclusion comes from in the official criteria, which documents the forms actually require, and the two narrow exceptions worth knowing.12

Who this is for

This guide is for mid to long term residents on work statuses, such as Engineer/Specialist in Humanities/International Services, and on family statuses, such as Dependent or Spouse of a Japanese National, who receive mental-health care and face a period-of-stay renewal.1 Renewal is the procedure for continuing the same status past its granted period without changing it, under Article 21 of the Immigration Control and Refugee Recognition Act.1

What visa renewal actually reviews

ISA permits renewal only when sufficient reason is recognized, judged by weighing the applicant's activity, residence situation, and necessity of stay together.2 The guidelines publish eight review lenses, and every one of them is about activity, conduct, money, or paperwork.

The eight lenses are: fit with the status activity, fit with landing-permission criteria, having actually performed the status activity, conduct, independent livelihood, proper employment and labor conditions, performance of tax and social obligations, and performance of notification duties.2 No lens examines health status, diagnosis, or treatment history, and mental-health treatment appears nowhere in the guideline text as a review item.2

The chart above maps the full logic of this article: treatment enters no review lens directly, while the two side channels worth watching run through employment continuity and conduct.

Tax, pension, and notification compliance

The compliance lens is the one readers control most directly. Applicants with tax obligations must perform them; non-performance counts negatively, whether through a criminal penalty for non-payment or through large or long-term arrears even without a penalty.2 Large or long-term arrears of legally required payments such as National Health Insurance premiums count the same way.2

ISA may base its decision on tax-performance information received from the relevant agencies on request.2 Mid to long term residents must also perform their card and organization duties: card-entry notifications, card validity renewal, reissuance, return, and employing-organization notifications.2

One related detail surprises readers. Officers ask to see health-insurance enrollment proof at the counter, using the My Number Portal qualification screen or a qualification notice since new card issuance ended on 2024-12-02.2 Inability to present it is explicitly not a ground for denial.2

Criminal record and continued eligibility for status

Conduct must be good as a premise. Conduct equivalent to deportation grounds, such as a criminal disposition, or administrative intolerables, such as brokering illegal work, counts as bad conduct even for a first offense.2 A criminal record connected to a mental-health episode falls under this lens exactly like any other record: the examiner weighs the disposition and the act, not the diagnosis.2

Continued eligibility means the applicant still falls under the status activity and has actually performed it.2 Long absence through re-entry permission without just cause counts negatively, as does failing to do the status activity at all.2 Livelihood must be independent and stable, assessed at the household level and free of public burden in daily life, with humanitarian reasons weighed where a burden exists.2

What renewal forms ask and do not ask

Renewal asks for paper that proves status, identity, taxes, and employment. The required documents follow the activity: the common items are the renewal application form, a photograph, and presentation of the passport and residence card, plus category-based employer documents.13

Assemble tax certificates before anything else

The residence-tax certificate is the document most likely to slow a work-status renewal, so request it from your municipality early and check that it shows both income and payment status.3

For work statuses, the tax evidence is the residence-tax taxation (or non-taxation) certificate and the tax-payment certificate covering one year of total income and payment status, issued by the municipality where you lived on January 1 (as of 2026-09-07; confirm current figures with the municipality).3 The documented required-documents lists contain no medical certificate, diagnosis, treatment history, or health-disclosure item for typical work-and-family renewal.13

The documents by status pattern

Status pages repeat the same pattern with category-based variation: large employers file lighter paper, smaller ones file more, but every variant stays within identity, residence, tax, and employment proof.3 The standard processing period runs 2 weeks to 1 month (as of 2026-09-07).1

The table above covers the only money in this procedure, and both rows date from the same September 2026 check (as of 2026-09-07; confirm current figures with the Immigration Services Agency).1

Why treatment records do not reach the examiner

The examiner's evidence is the application form plus the status-based documents listed above, and no treatment-record channel exists in either the procedure page or the guidelines.12 Clinics and hospitals do not report visits to ISA, and the guidelines give the examiner no lens that would use such records if they arrived.

A hospital stay appears on the procedure page in exactly one place: as an example of special circumstances that allow filing earlier than 3 months before expiry, with supporting materials through the regional bureau.1 Early filing during treatment is a facilitated path, not a mark against the applicant.1

Rare exceptions that can touch a renewal

Two boundary conditions deserve honesty without alarm. Neither makes treatment itself a renewal factor, but each is a channel through which events around an illness can reach the review.

Extended leave and the unemployment window

Where extended hospitalization and leave keep a worker away from work for months, the renewal risk runs through the activity-continuity and livelihood lenses, not through the treatment.2 The question the examiner can ask is whether the person still performs the status activity and maintains a stable livelihood, never what they were treated for.2

The status page warns that failing to perform the status-based activity can lead to revocation of status.3 The detailed unemployment-window mechanics belong to the pillar's visas-immigration procedures coverage; this guide states only the channel so readers know which procedure to read next.2

A criminal record arising from an incident

Any criminal disposition connected to a mental-health-related incident is reviewable under the conduct lens exactly like any other disposition.2 The review weighs the act and its disposition, the same test applied to every applicant with a record.2 No separate health-based inadmissibility or renewal bar for typical work-and-family statuses appears in the procedure or the guidelines.12

Practical takeaway: the real risk direction

The practical implication reverses the fear that keeps some residents from booking an appointment. The documented review criteria punish neither the visit to the 精神科 (seishinka, "psychiatry") clinic nor an ongoing prescription; they punish the downstream events that untreated illness makes more likely.12

Seeking treatment does not put the visa at risk

Because no review lens examines treatment and no form asks for it, seeking treatment does not itself create a renewal disadvantage under the published criteria.12 Keeping appointments, staying insured, and staying at work is the renewal-friendly path, and treatment supports all three.

Do not let fear of disclosure delay care you need

No renewal form asks about mental-health treatment, so a decision to avoid the clinic buys no visa safety at all.13 It only trades a documented non-risk for the documented risks of untreated illness described next.

Untreated illness causing job loss is the more real risk

Job loss caused by untreated illness reaches renewal through the same activity-continuity and livelihood lenses described above, which makes it the more realistic channel of harm.2 A worker who loses their position and stays out of status activity faces questions no treated and employed resident ever meets.2

Employees who need certified medical leave should also know that a separate sickness-allowance mechanism exists for insured employees, with its own eligibility rules covered in this pillar's sickness-allowance coverage. This guide asserts no figures for that benefit and sends readers to that coverage for mechanics.

Good to know

Renewal timing still matters even when health does not

File inside the filing window even while in treatment. Applications run up to the expiry date, and from 3 months before expiry for holders of 6-month or longer periods.1

Hospitalization counts as an accepted special-circumstances reason for filing earlier than that, with supporting materials submitted through the regional bureau.1 Treatment and timely filing go together; neither replaces the other.1

Keep tax and pension payments current during leave

Arrears built up during leave count the same as any other arrears. Large or long-term arrears of taxes or legally required payments such as National Health Insurance premiums count as negative elements even without a criminal penalty.2

Leave does not suspend the compliance lens. Setting up automatic payment or a household member's reminder before a long admission prevents a paperwork problem from joining a health problem.2

A hospital stay does not pause your notification duties

Card and organization notifications keep running during treatment. Card-entry, reissuance, return, and employing-organization notifications form a standalone review lens with no treatment exception stated.2

Readers admitted for a long stay should confirm that address, employer, and card-validity paperwork stays current, asking family or a cohabitant for help where the procedure page allows a relative to appear with a medical certificate.12
