Maternity and Childbirth Allowances

Overview

Japan runs two childbirth benefits side by side through the health insurance system.1 The 出産手当金 (shussan teatekin, "maternity allowance") replaces income an enrolled employee loses while on pre- and post-natal leave.2 The 出産育児一時金 (shussan ikuji ichijikin, "childbirth lump-sum payment") offsets the cost of the birth itself, per child, for insured persons and their dependents alike.3

The split matters because the two benefits have different gates.1 The allowance is employees-only: National Health Insurance (NHI) enrollees cannot claim it.2 The lump sum is universal across public coverage, so NHI members claim it the same way employees do.4

Who can receive the maternity allowance

Only the insured employee herself can receive the allowance.2 It is paid when she takes leave for childbirth and receives no salary for the leave days.2 Filing destination follows the insurer: the Kyokai Kenpo branch covering the employer, or the employer's own health insurance society for society members.25

Enrolled-employee requirement

Working on your resignation day ends post-loss eligibility

Leaving while on leave, including paid leave, preserves the post-loss claim. Clocking in on the resignation day itself disqualifies it, even with a year of enrollment behind you.2

How much and for how long

The paid window runs from 42 days before the expected birth date through 56 days after the actual birth, covering only days actually taken as leave.2 Multiple pregnancy extends the pre-natal start to 98 days before the expected date.2 The birth day itself counts inside the pre-natal portion.2

The 42-day plus 56-day window

The daily-amount formula

Each paid day is worth two-thirds of the daily standard remuneration.2 The base is the 12-month average of 標準報酬月額 (hyōjun hōshū gekkyaku, "standard monthly remuneration"), divided by 30, times two-thirds, with rounding at each step.2 Kyokai Kenpo's worked example averages 160,000 JPY over 6 months with 180,000 JPY over 6 months to 170,000 JPY, divides to about 5,670 JPY, and pays 3,780 JPY per day.2

Short-tenure cases use a floor rule (as of 2026-09-07; confirm current figures with Kyokai Kenpo).2 Where the 12 months before the start date are incomplete, the calculation uses the lower of the actual average and the all-insured average reference of 320,000 JPY for starts on or after 2025-04-01 (as of 2026-09-07).2

Overlap with salary and sickness allowance

Amount per child

Eligibility needs two facts: enrollment in Japanese public medical insurance at birth, and pregnancy at 4 months (85 days) or more.4 Method and place of birth do not matter once those hold.4 The 産科医療補償制度 (sanka iryō hoshō seido, "obstetric medical compensation system") participation of the facility decides which of the two current amounts applies.3

Normal delivery and C-section settle differently

Normal delivery settles against the lump sum, not the 70/30 insurance mechanic. A C-section counts as insurance treatment instead, so the high-cost medical expense cap can apply on top.3

How the money reaches the clinic

How this relates to childcare leave pay

Childcare leave pay, the 育児休業給付金 (ikuji kyūgyō kyūfukin, "childcare leave benefit"), belongs to a different system.2 It runs through employment insurance with its own eligibility and its own application at the Hello Work office, not through the health insurer. Separate system, separate application

File the allowance with the enrolled Kyokai Kenpo branch or health insurance society, never with Hello Work.25 The allowance window ends 56 days after birth under the health insurer, while childcare leave pay starts under employment insurance; rates and durations for that side sit outside this article's verified scope and are not stated here.2

How to apply

The allowance uses the 健康保険出産手当金支給申請書 (the maternity allowance claim form), completed across three parties: the employee, the employer, and the attending physician or midwife.25 The lump sum uses the 健康保険出産育児一時金支給申請書 (the lump-sum claim form), with related internal-payment and post-loss certificate forms where applicable.3 Both go to the enrolled branch, and the benefit rules are identical nationwide; only the branch counter differs.23

Deadlines

The lump sum rarely covers the full birth bill in large cities

Late birth extends the paid window, while early birth shortens the total
