English-Speaking Mental Health Providers

English-speaking mental health providers in Japan cluster around a small set of documented channels rather than a single booking site.123 This guide maps those channels: TELL for counseling and crisis support, the IMHPJ directory for therapists, embassy-published lists for leads, and the insurance-accepting and private-pay clinic tiers.

Overview

Japan's clinics and hospitals operate almost entirely in Japanese, and mental-health vocabulary challenges even conversational speakers. An English-speaking provider removes that barrier where misunderstanding costs the most: assessment, consent, and ongoing care.2

This guide covers foreign residents seeking care in English. It separates talk therapy from psychiatric medication, states what public insurance does and does not cover, and gives dated fee bands so the private-pay reality is visible before booking.45

Insurance rules are uniform nationwide, but English availability is not. Tokyo holds the densest coverage, with thinner bilingual options in Osaka, Kyoto, and elsewhere, so every entry below is a lead to confirm rather than a guarantee.26

Where to start: TELL for counseling and crisis support

TELL Japan is a certified nonprofit serving the international community with counseling centers and a separate crisis line. Start here when the need is urgent or when no other English path is confirmed yet.17

TELL Counseling centers and telehealth

TELL Counseling provides confidential face-to-face and distance counseling and psychotherapy for adult individuals, couples, families, children, and adolescents.1 Centers stand in Tokyo and Okinawa, with telehealth available anywhere in Japan.1

The team provides services in English, Japanese, and Cantonese, with broader counseling coverage also listing Spanish, Portuguese, Urdu, and Hindi.12 Every therapist and psychologist is licensed in Japan or overseas, or directly supervised by a qualified clinician.1

TELL therapists do not prescribe medication. They work with the client's physician or refer to a psychiatrist when medication is indicated.4

The Tokyo office takes online counseling Monday to Friday from 9:00 to 16:00, with limited evening and in-person slots, and clients may wait until a suitable provider opens (as of 2026-09-07; confirm current hours with TELL).1 The Okinawa office takes in-person and online sessions Monday to Saturday with limited availability and a possible waitlist (as of 2026-09-07).1

Book TELL counseling by appointment, not as a walk-in

General counseling sessions run by appointment only with no same-day appointments. Request intake through the Tokyo or Okinawa appointment form and expect a waitlist reply with the current wait time.1

TELL Lifeline hours and limits

The TELL Lifeline at 0800-300-8355 is toll free and open to everyone in Japan who speaks English, free of charge.7 Callers need not be suicidal to reach out. Trained volunteer support workers listen, help callers talk through concerns, and can connect them to community services.7

Hours split across phone and chat. Phone and chat together cover Saturday 09:00 through Monday 23:00 continuously, Tuesday to Thursday 09:00 to 23:00, and Friday 09:00 to 02:00, with chat available daily from 16:00 (as of 2026-09-07; confirm current hours with TELL).7 Check the weekly hours page before calling to see whether phone or chat is live.7

The Lifeline is not an emergency service. In an emergency, call 110 for police or 119 for ambulance.7 Only people located in Japan may use the service, and the minimum age for chat is 13.7

Finding a therapist or counselor in English

Talk therapy in English sits almost entirely in the private tier. Counseling centers and independent therapists charge session fees directly, with payment out of pocket, through private insurance, or through an employer assistance program.425

The カウンセリング (kaunseringu, "counseling") label here means talk therapy distinct from an insured psychiatric visit. Public insurance does not convert a counseling session into a covered medical visit.42

Using the IMHPJ directory

The International Mental Health Professionals Japan (IMHPJ) runs a searchable directory of licensed and accredited professionals working in Japan.3 It is a voluntary professional network, and individual members remain responsible for their own actions under its Code of Ethics.3

Clinical members hold a master's degree or higher plus at least two years of full-time supervised clinical experience. Associate Clinical members hold the degree and work toward that experience under supervision.3

Search by accepting-clients status, location, and language. Filters cover Tokyo, Kansai, Okinawa, online, and other areas, with English among the language tags.3 Directory membership signals a training tier, not a booking guarantee, so confirm accepting status and language with the professional directly.3

Private practices and counseling centers

Tokyo Mental Health provides talk therapy for individuals, couples, and families plus psychological evaluations at its Shintomi Therapy Office in Tokyo, in Okinawa, and online.5 It is not a medical clinic and cannot prescribe medication or issue medical documentation at its own offices.5

Psychiatry runs through its partner, American Clinic Tokyo, where Dr Andrew Kissane holds a psychiatry clinic for medication and medical documentation needs.56 Therapy and psychiatry therefore mean two bookings at two sites, not one combined visit.5

Standard individual therapy costs 24,000 to 28,000 yen tax included for 60 minutes (as of 2026-09-07; confirm current figures with Tokyo Mental Health).5 Discounted self-pay sessions cost 14,500 to 19,000 yen tax included for a standard 50-minute consultation (as of 2026-09-07).5 Tokyo Mental Health cannot accept Japanese National Health Insurance and issues invoices and receipts for private-insurance reimbursement claims instead.5

The Shintomi office opens 09:00 to 20:00 seven days including holidays, with the earliest appointment at 09:00 and the latest at 19:00 (as of 2026-09-07).5 At high demand a waiting list applies, with roughly four to six weeks to schedule once waitlisted, or about two weeks when no waitlist runs (as of 2026-09-07).5

The Tokyo Meguro Counseling Center consists of Western-trained therapists serving the international community from the Shibuya-Ebisu area near Nakameguro, plus online sessions, in English and Japanese.8

Expect the full fee for late cancellations

Tokyo Mental Health charges the full session amount for cancellations with less than 24 hours notice and for missed appointments. TELL charges 11,000 yen tax included under the same window, or the lower agreed fee for sliding-scale clients (as of 2026-09-07).45

Finding an English-speaking psychiatrist

The 精神科 (seishinka, "psychiatry") path covers diagnosis and medication. Psychiatrists work from hospitals or clinics, focus on brief biomedical check-ins and prescriptions, and normally do not provide talk therapy.2 The initial intake runs longer than follow-ups.2

Not all medications prescribed overseas are licensed in Japan, so the same prescription may not be available after arrival.2 In Tokyo, non-Japanese patients needing inpatient psychiatric care are generally provided care at Matsuzawa Hospital in Setagaya.2

Insurance-accepting psychiatric clinics

Some English-capable psychiatric clinics accept Japanese public insurance. Residents then pay the standard copay share at the counter rather than the full private fee.910

Documented examples in Tokyo include Yotsuya Yui Clinic, Mejiro Sola Clinic, Kitasando Mental Health Clinic, and Kokoro Terrace Park Side Clinic. Reported initial-visit copay bands sit around 2,500 to 5,000 yen under national insurance (as of 2026-07-28; confirm current figures with each clinic).10

Yotsuya Yui covers psychiatric consultation with counseling and testing in several languages including English. Mejiro Sola covers psychiatry and psychosomatic medicine in English, German, and French.10 Insurance acceptance and English coverage vary by clinic, so confirm both before visiting.10

Private-pay psychiatric consultations

Premium English-speaking psychiatric consultations that do not accept public insurance charge full private fees. American Clinic Tokyo, the psychiatry partner of Tokyo Mental Health, operates in this tier and cannot accept Japanese National Health Insurance.5

Indicative private-pay psychiatry sits on the order of 15,000 yen for a short consultation of about 15 minutes, with initial consultations higher (as of 2026-09-07; confirm current figures with the clinic).11 Practitioner sources describe this band inconsistently, so treat it as an order of magnitude rather than a fixed schedule.11

Private insurance may reimburse part of private psychiatric costs depending on the policy. Tokyo Mental Health provides invoices and, where needed, a short insurer letter about the service received.5

Bilingual networks in Osaka and Kyoto

Outside Tokyo there is no single dominant English-speaking mental-health clinic chain in the sources. Coverage appears as hospital departments plus directory entries and online counseling.36

The UK facility table lists Nippon Life Hospital in Osaka (Nishi-ku) with English and Japanese and psychiatry and mental-health specialisations. It lists Koseikai Takeda Hospital in Kyoto with English, Chinese, Korean, and Japanese support.6 The IMHPJ directory adds Kansai-based and online English-speaking professionals searchable by Kansai location plus the online filter.3

Embassy-published provider lists

Embassies and consulates publish provider lists for their citizens in Japan. Each list carries an explicit non-endorsement disclaimer and is compiled from publicly available information, so treat every entry as a lead to confirm rather than a guarantee.69

United States embassy lists

The United States Embassy and Consulates maintain a Medical Assistance page linking to city and region doctor lists, including Tokyo and the surrounding area.12 The page states that inclusion is not an endorsement and that the department assumes no responsibility for a provider's ability or quality of service.12

The same site points readers who need counseling or psychiatric services to a separate counseling list and to the Tokyo English Lifeline.12

United Kingdom and Australian lists

The UK Foreign, Commonwealth and Development Office publishes a table of medical facilities across Japan, updated 2024-10-31, with address, telephone, languages, specialisation, and accreditation for each entry.6 The list states it is not exhaustive, is subject to change, and carries no endorsement; it also cross-refers readers to the JNTO medical institution guide.6

Mental-health-relevant entries include Tokyo Mental Health in Chuo-ku for mental health and psychotherapy, American Clinic Tokyo in Minato-ku for general medicine with psychiatry and psychology, and Roppongi Clinic in Minato-ku for mental health, psychiatry, and psychotherapy, all private.6

The Australian Embassy Tokyo takes a different approach. Instead of its own doctor roster, its medical page points readers to the JNTO hospital locator for facilities with English-speaking staff.9 The same page states that Australia has no reciprocal health-care agreement with Japan, and that Australians resident for more than three months must register for national health insurance at the local city office.9 It also lists AMDA, Himawari, TELL, and Japan Healthcare Info as further services.9

Costs and insurance: the pricing reality

English-speaking mental-health care often means private-pay or partial insurance. Psychiatry can fall under public insurance at accepting clinics, while counseling almost never does.425

What insurance covers and what it does not

TELL Counseling services are not covered by Japan National Health Insurance.4 Tokyo Mental Health services are not medical services in Japan for insurance or tax purposes and cannot accept it either.5

Psychiatric consultations and prescribed medication at insurance-accepting clinics fall under public insurance with the standard copay share. Talk therapy and counseling do not, and are paid out of pocket or claimed against private insurance.42510

TELL is a provider for Tricare (ISOS) and AFSPA, with direct billing for Tricare Prime and AFSPA subject to deductibles and copays. All other private-insurance clients pay upfront and submit receipts for reimbursement (as of 2026-09-07; confirm current figures with TELL and the insurer).4

Check EAP coverage before paying out of pocket

Some employers cover a fixed number of counseling sessions through an Employee Assistance Program at no cost to staff. TELL acts as a provider for major EAP companies and holds direct contracts with several multinationals in Tokyo. Ask human resources before booking.42

Typical fee bands and sliding scales

TELL uses a sliding-scale structure by family and household income for clients without insurance coverage, applying to households with monthly income of 900,000 yen or less and excluding third-party-paid sessions (as of 2026-09-07; confirm current figures with TELL).4 The standard fee is 24,200 yen tax included per 53-minute session (as of 2026-09-07).4

Tokyo Mental Health's discounted self-pay band is 14,500 to 19,000 yen tax included per 50-minute individual session (as of 2026-09-07).5 Secondary compilations place general English private therapy around 11,000 to 25,000 yen per session, with couples and family sessions higher, but report the band inconsistently (as of 2026-09-07).11

Therapy receipts are not tax-deductible on a Japanese tax return, per Tokyo Mental Health.5 First-time, translation, and documentation fees can sit outside the session fee at some English-speaking clinics, so ask for the full estimate before booking.5

English-access reality check: what to do when nobody speaks English

When no English-speaking mental-health provider is reachable, bring interpretation to the visit instead of postponing care. Medical information services bridge the gap by phone and can suggest options with non-Japanese language support, though no service guarantees a given language on the day.2

AMDA offers nationwide toll-free telephone consultation at 0120-339-266 and 03-6233-9266, open 10:00 to 16:00 Monday to Friday in English and other languages (as of 2026-09-07; confirm current hours with AMDA).2 The Tokyo Navii line at 03-5285-8181 covers Tokyo from 09:00 to 20:00 daily in English, Mandarin, Korean, Spanish, and Thai (as of 2026-09-07).2

The JNTO medical institution guide, referenced by both the UK and Australian pages, provides a searchable list by location and language.69 The sequence that works is shortlist through an official channel, call ahead to confirm English on the day, then visit.

On the call, ask two questions: whether an English-speaking provider is on duty for the visit slot, and whether the visit falls under insurance or private-pay with the full fee stated upfront.26 Do not assume an English website means English on the day; the UK list itself notes at least one facility where English depends on shift.6

Good to know

Counseling fees stay private-pay even with an insurance card

Holding National Health Insurance or Employees Health Insurance does not convert a counseling session into a covered visit. Both TELL and Tokyo Mental Health state their counseling cannot be billed to public insurance.45

Budget the standard session fee before booking: 24,200 yen at TELL, or 24,000 to 28,000 yen at Tokyo Mental Health, with sliding-scale or discounted relief only where income or self-pay terms qualify (as of 2026-09-07; confirm current figures with each provider).45

A psychiatry visit is short and medication focused

Do not book a 精神科 visit expecting a 50-minute conversation. Psychiatric clinics generally offer brief biomedical check-ins centered on symptoms and prescriptions, not talk therapy.2

Use psychiatry for diagnosis and medication, and a separate counseling booking for ongoing talk therapy. The two tiers complement each other but do not substitute.25

Confirm English on the day, not just on the website

Listing language labels describe capability, not the daily roster. Call ahead and ask whether the English-speaking provider is on duty for the exact visit slot.6

Arriving unconfirmed risks rescheduling at reception or proceeding through interpretation that could have been arranged in advance. Where the booking phone is Japanese-only, ask AMDA or the Tokyo line to bridge the call.2

Outside Tokyo, expect fewer choices and longer waits

Dedicated English-speaking mental-health clinics concentrate in Tokyo. Kansai coverage appears mainly as hospital departments and IMHPJ directory entries, with online counseling filling part of the gap.36

Start the search earlier outside the capital, include online and telehealth filters, and keep the embassy lists plus the JNTO guide open alongside the directory.369
